The Process

Five steps toward beginning services.

Every situation is different, but most new services follow this general path. Your case manager or lead agency remains responsible for eligibility and authorization decisions.

    Begin with your lead agency

    Contact your county or Tribal human services office if you have not yet completed an assessment. The lead agency determines eligibility and helps identify appropriate services.

  2. 02

    Build your person-centered plan

    Work with your case manager or care coordinator to identify the services, goals, schedule, and support approach that fit your assessed needs and preferences.

  3. 03

    Choose Ontime Health Care

    Tell your case manager you are interested in Ontime, or contact us directly. We can answer general questions and explain what information is needed for a referral.

  4. 04

    Submit the referral

    The individual, family, guardian, case manager, or another authorized professional may send basic referral information through our online form or by phone.

  5. 05

    Complete intake and coordinate the start

    We review the authorized service, discuss routines and preferences, confirm availability, and coordinate an appropriate start date with the person and support team.

Can I contact Ontime before services are authorized?

Yes. We can answer general questions and explain the referral process, but services cannot begin until eligibility, authorization, availability, and required intake steps are confirmed.

Who can submit a referral?

A person seeking services, family member, guardian, case manager, care coordinator, or another authorized professional may start a referral.

Does submitting the form guarantee services?

No. A referral begins the conversation. Ontime must review the requested service, authorization, staffing, location, and other intake requirements before confirming a start.

Should medical records be sent through the website form?

No. Do not submit diagnoses, Social Security numbers, medical records, or other sensitive health information through the website form. Ontime will explain a more appropriate process if additional documentation is required.
