Ontime Health Care provides 245D licensed services for people who need support at home, at work, or in the community. Each service is shaped by the person's assessed needs, preferences, goals, routines, and person-centered support plan.
We coordinate with individuals, families, case managers, lead agencies, and other members of the support team. This collaboration helps create a smooth start to services and dependable support that can grow with the person over time.

Service Overview
Basic Support Services
Basic Support Services help people meet everyday needs, maintain important routines, and remain safe at home and in the community. Depending on the authorized support plan, services may include 24-hour emergency assistance, adult companion services, homemaker support, night supervision, respite care, individual community living support, and individualized home supports. Respite may be provided in or outside the home, giving primary caregivers time to rest while dependable support continues.

How to begin 245D services.
Access begins with an assessment and service authorization through your county or Tribal lead agency. Once that is in place, Ontime will help coordinate a smooth start to care.

Request an assessment
Contact your county or Tribal human services office and ask for a MnCHOICES assessment to determine eligibility for home and community-based services.

Choose services with your case manager
Review your assessed needs, select the services that fit your goals, and have them authorized in your person-centered support plan.
