Individualized Home Supports help a person build or maintain the skills and routines that support greater independence at home and in the community.

Service Overview

Support centered on the person’s daily life and goals.

Support is guided by the person’s assessed needs, preferences, goals, and authorized support plan. The focus may be maintaining important routines, developing practical skills, participating in the community, or strengthening confidence with everyday responsibilities.

Ontime coordinates with the person, family or guardian, case manager, and other members of the support team so expectations are clear and services remain centered on what matters to the individual.
